I accept the following terms for my own entry. I also accept the terms on behalf of any other participants I am entering in this event, all of whom have been provided with a copy of these terms and accept them:

Athlete Waiver

Potential Risks

• I declare that I have no known medical conditions that prevent me from competing at EAST ROX.
• I declare that if I have any medical conditions, that I have received appropriate medical advice prior to participation in the event.
• I delcare that my physical condition does not preclude me to participate in the event.
• I understand that I take personal responsibility for my health and injury prevention during the event. I understand the inherent potential risks and hereby release Go Train East Rocks and any associates, partners, consultants, contractors, interns, and/or advisors from all liabilities that may occur during my participation. In the unlikely event of physical injury during my participation, Go Train East Rocks and/or those previously listed will not be liable for any medical treatment or monetary compensation.
• I realise I have the right to, and should, discontinue any exercise at any time due to excessive discomfort related to dizziness, lightheadedness, muscle or joint pain, chest pain, difficulty in breathing, or any other reason. I also assume responsibility to report immediately any of these previous conditions.
• Furthermore, I am aware that it is my responsibility to receive clearance from my physician prior to participation in vigorous activity and that any previous or current health-related problems or injuries will be disclosed to Go Train East Rocks prior to the start of the event.

I have read this form completely and have been fully informed of the risks associated with performance training. I realise that all participation in the event is voluntary and by signing this document hereby freely consent to participate.

Release Declaration

By electronically signing and/or confirming this statement of declaration in the form of opting-in, you (hereinafter also referred to as "I") hereby declare the following:

1. Consent and Voluntariness
   I have carefully read the organizer's limitation of liability and understood that I carry the inherent risks of participation as an athlete. I herewith agree and acknowledge that participation in the event is entirely voluntary and that due to the typical dangers of sports I bear a large part of the risk connected to the Event myself.
2. Health
   I am physically able to participate in the Event and have no knowledge or reason to believe of any circumstance that would compromise my safety or health by attending the Event. I will carefully monitor my state of health and my physical fitness in the period between he conclusion of my participation agreement and the start of the competition, and I will refrain from participating as soon as there are any indicators that endanger my safety or health through participation. Should health problems become apparent during the competition, I will inform the staff and, if necessary, stop competeting. I will follow any instructions given by the security and/or medical staff during the competition.
3. First Aid
   I consent to the provision of first aid and other medical treatment in the event of injury or illness (including but not limited to cardiopulmonary resuscitation and use of an automated external defibrillator) and hereby exonerate the organizer and release them from any liability or claims arising from such treatments. I am aware that I can withdraw from the event at any time for my own protection and the protection of others.
4. Filming and Photography Release
   I hereby grant permission for EAST ROX to record and use images of myself prior and during the event.
   I authorise EAST ROX to take photographs and film me. I authorise EAST ROX to use my image for the purposes of promoting across media platofrms (social, website, mailers).
   I authorise EAST ROX to edit, alter and copy the images for social media and advertising p urposes. I agree that media is all intellectual property rights of EAST ROX.
   I understand that I can withdraw my consent at any time by contacting EAST ROX.
